A company is looking for a Care Navigation Guide to join their
Clinical Support Services team.
Key Responsibilities
Engage with providers to curate personalized care options for
members
Collaborate with nurses on complex cases and document relevant
information
Monitor trends and communicate insights to leadership for process
improvement
Qualifications
1+ years of experience in care coordination and navigation in a
healthcare setting
1+ years of experience using a Chromebook and Google Workspace
Bilingual in Spanish and English (preferred)
Experience working in a collaborative environment (preferred)
